Always report the breakdown. Two patients with the same total can be in very different states.
This is a reference implementation of a published formula, provided for use by qualified healthcare professionals. It does not diagnose, does not account for the individual circumstances of any patient, and is not a recommendation for treatment. Confirm every result against your own institutional guidance before acting on it. Clinical judgement always takes precedence over a calculator.
About this calculation
The Glasgow Coma Scale describes level of consciousness across three components. The breakdown carries more information than the total — two patients with the same total can be in very different states — so it is always reported alongside it.
Formula
Eye (1–4) + Verbal (1–5) + Motor (1–6), total 3–15

What this does not account for
- Always report the components, not just the total. E1V1M5 and E3V2M2 both total 7 and describe very different patients.
- The verbal score cannot be assessed in an intubated patient; convention is to record V as 1T and treat the total as incomplete.
- Sedation, intoxication, paralysis and aphasia all confound the score independently of brain injury.
