This is a reference implementation of a published formula, provided for use by qualified healthcare professionals. It does not diagnose, does not account for the individual circumstances of any patient, and is not a recommendation for treatment. Confirm every result against your own institutional guidance before acting on it. Clinical judgement always takes precedence over a calculator.
About this calculation
The HEART score estimates the risk of a major adverse cardiac event within six weeks in patients presenting to the emergency department with undifferentiated chest pain. It was built for that setting specifically.

What this does not account for
- Derived and validated in emergency department chest pain. It does not apply to a patient with a confirmed STEMI, who needs reperfusion rather than risk stratification.
- The History component is a subjective judgement and is the main source of variation between assessors.
- Troponin scoring was derived with conventional assays. High-sensitivity assays shift the thresholds, and several modified pathways exist.
- A low score supports an early discharge pathway in combination with serial troponins and local protocol — not on its own.
